Antioxidants (i.e. vitamins C and E and glutathione) as well as protein thiols are essential to protect cells from damage due to free radical stress. This study was undertaken to assess the levels of plasma antioxidant enzymes and erythrocyte protein thiols in infants and children with various forms of congenital heart disease. Blood samples were taken from 27 patients ranging in age from 0 to 17 years. Antioxidant enzymes were measured by spectrophotometric methods and protein thiols by the Ellman's method. A reduction in levels of superoxide dismutase (p=0.000) and catalase (p=0.005) was seen in patients with cyanotic lesions compared to those without cyanosis. Erythrocyte protein thiol levels were reduced in patients with cyanotic lesions (p=0.011). In cyanotic patients, there was a negative correlation between mean oxygen saturation and levels of erythrocyte catalase (r=-0.61, p=0.012). In conclusion, reduced activity of antioxidant enzymes was observed in patients with cyanotic lesions, suggesting that free radical reactions are increased in such patients. This study suggests the presence of subclinical oxidative stress in patients with congenital heart disease.
